Get the 2027 final rule, decoded
CMS releases the final 2027 fee schedule in early November. Get one short email on what changed and what it means for your pay. No spam.
Customize a sample clinician
No report handy? Make a fictional clinician from real 2024 Medicare patterns for any specialty, then explore every feature.
Annual wRVU and rate start from national figures for the specialty and production level. The code mix comes from 2024 Medicare billing. Edit either to match the clinician you have in mind.
Department view
Select several reports at once to compare clinicians side by side. Each is read on this device and nothing is kept.
Private by design
Your file is read by code running on your own device. No upload and no server. Nothing is saved unless you choose to keep a code summary on this device.
Every specialty, built by a surgeon
Peer comparisons and a best-hour ranking for every Medicare specialty, from family medicine to neurosurgery, plus surgeon-built checks for procedures and add-on codes.
Questions, not billing advice
Findings are framed as questions to review with your coding team. Only bill what you did and documented.
Your best hour
What each hour of your time earns, by the work you do most. Edit the minutes to match your real times and the ranking updates.
Times are your time per case or visit, including turnover. Average pace uses typical case times, common visit slots, or CMS physician time. Add-on codes and imaging guidance such as fluoroscopy are left out of the ranking because they happen during another procedure.
See what shifting a few hours a week, or adding hours, would be worth.
Your contract
Enter your terms once; everything below updates.
Your pay is your collections, topped up so it never falls below the guaranteed rate for every wRVU. "Taken from collections first" is any overhead or department tax. Assumes the top-up is settled over the full year and applies to every wRVU.
Most contracts pay each tier's rate only on the wRVUs inside that tier. Leave "salary covers" at 0 if it doesn't apply. If your contract states a base salary, enter it and the bonus is paid on wRVUs above the salary line. Production above the salary line is shown as bonus.
Call pay, quality bonus, stipends and anything else paid outside wRVUs. Choose how each is paid and how often.
Private practice: ancillary income and ASC distributions are usually paid as yearly or quarterly distributions. Enter your yearly share.

How you compare with your peers
Your report against 2024 Medicare billing for clinicians in your specialty. Bars show the middle half of peers, the line is the median, the dot is you.

Questions for your coding team
Add-on codes your peers capture more often
Companion codes for your common procedures
High-yield codes that can go with procedures you do often, when the extra work is done and documented.
Add your own companion pair
Your pairs are kept on this device and checked against every report you open.
Shared visits and team credit
Your best OR hour
wRVU per hour of your time in the room (wheels-in to wheels-out plus turnover). Values use what your report credited where available. Minutes are typical starting points — replace them with your own and the list re-sorts. Faded rows aren't in your report.
Right column = minutes per case. Ignores 90-day global visits, which add more time to revisions and fractures.
What the 2027 Medicare proposal costs you
CMS proposed cutting work RVUs for primary hips (27130: 19.11 → 15.37) and knees (27447: 19.11 → 15.94). Most wRVU contracts credit the CMS table for every payer, so the cut applies to commercial cases too. Co-surgeon lines are scaled. Proposed values may change in the final rule.

Compare an offer and prepare to negotiate
Enter another contract or a renewal offer. It's valued against your current production.
Tiers are "starting wRVU:rate" pairs, for example 0:62, 8000:70, 12000:78. Pay is the higher of the guarantee and production.

Ask your report
Ask anything about your numbers. Only code totals and the figures on this page are sent to Claude, never your file or any patient details.
